What Hospice Care Includes
Our hospice team supports you at home, in assisted living, or in a nursing home. Services may include: nursing and physician oversight, medications related to comfort, medical equipment and supplies, social work support, chaplaincy, bath aides, therapy consults, and trained volunteers. Care is tailored to your goals and values.
Four Levels of Hospice Care
There are four levels of hospice care with 24/7 Care At Home.

Routine Care
Most care happens at home with regular visits.

General Inpatient Care
Short-term care in a facility if symptoms cannot be managed at home.

Continuous Care
Temporary round-the-clock nursing at home during a crisis.

Respite Care
Short stay in a facility to give caregivers rest.
Who is Eligible
A doctor certifies a life expectancy of six months or less if the illness follows its usual course.
You choose comfort-focused care rather than curative treatment for the terminal illness.
You receive care from a Medicare-certified hospice provider.
Coverage note: Medicare Part A covers hospice for eligible patients. Some costs may apply, such as a small copay for medications at home and up to 5% for respite. Room and board are generally not covered.
